Exceptional Achievement

By submitting this application, you acknowledge and agree to the following:

1. Application Review

Submission of this application does not guarantee engagement, placement, or assignment. All applicants are subject to review, screening, and approval at the sole discretion of Exceptional Achievement.

2. Independent Contractor Status (1099)

If selected, you will be engaged as an Independent Contractor (1099) and not as an employee.

You understand and agree that:

• You are not an employee of Exceptional Achievement.

• You are responsible for your own federal, state, and local taxes.

• You will receive a Form 1099-NEC for compensation paid.

• You are not eligible for employee benefits (health insurance, retirement, workers’ compensation, unemployment insurance, etc.).

• You maintain control over your availability and acceptance of assignments.

A separate Independent Contractor Agreement will be required prior to onboarding.

3. Background Check Authorization

Exceptional Achievement prioritizes student safety.

By submitting this application, you authorize Exceptional Achievement to conduct a thorough background screening, which may include:

• National criminal history check

• State and county criminal records search

• Identity verification

• Sex offender registry check

• Review of publicly available records

Engagement is contingent upon satisfactory results. Failure to pass screening will result in disqualification.

4. Accuracy of Information

You certify that all information provided is accurate and complete.

Falsification, omission, or misrepresentation may result in:

• Immediate removal from consideration

• Termination of contract if already engaged

5. Professional Conduct & Confidentiality

If engaged, you agree to:

• Maintain strict confidentiality of student and family information

• Comply with all applicable federal and state laws regarding student privacy

• Uphold ethical instructional practices

• Promote equitable and inclusive learning environments

6. No Guarantee of Assignments

Exceptional Achievement does not guarantee a minimum number of students, sessions, or earnings. Assignments are based on client demand, subject expertise, performance, and availability.

7. Agreement

By checking the box on Jotform and submitting the application, you confirm that:

• You have read and understood these terms

• You consent to background screening

• You acknowledge Independent Contractor (1099) status
